  "works": "Re-checked a different way: drove the official TypeScript SDK (codex-sdk 0.166.0) from Node instead of the exec command line, offline against a local stand-in model through the SDK's base URL option, with new inputs, a self-written local MCP server, slow streaming, and multi-turn runs resumed across SDK instances. Runs, multi-turn resume, streamed events and command items with exit codes all worked through the SDK.",
  "breaks": "Reproduced through the SDK: a false 'All tests passed' after a command exited 3 still resolves the run normally with that text as the final response (the failed command item is visible, but nothing checks it). An unreachable model server never fails the turn (no failure within 120 s, only repeated reconnecting events with growing backoff) until the caller aborts. Commands blocked by the sandbox, including refused escalations, are missing from SDK items, so the caller only sees the model's message (public issue #47433). HTTP 500 from the model server surfaced as a 'high demand' message after 30 requests. New: the SDK's on-request approval policy is ignored (escalation is still refused as if approval were never allowed). MCP tool calls fail by default with an approval error unless each tool gets approval_mode approve in config, which the SDK docs don't mention, and the error item for an MCP error result is null. With a custom base URL every turn first retries WebSockets 5 times (about 6 s) and emits top-level error events even when the turn succeeds. A missing Codex home folder makes every SDK run fail.",

  "test_method": "Lab check (independent re-run a different way): official TypeScript SDK from Node 24 in an offline sandbox with a throwaway home folder, a local stand-in Responses API server, new failure inputs (failing test script, schema mismatch, escalation request, writes outside the workspace, slow stream, closed port), a local stdio MCP server, and multi-turn plus resume across SDK instances. "scoring_note": "Confirmed partial 3/5: the unchecked false-success, never-failing unreachable server and hidden sandbox denials reproduced through the SDK too, plus an ignored on-request approval policy, MCP tools blocked by default without per-tool config, noisy WebSocket retries and a hard failure when the Codex home folder is missing.",
  "security_flags": [
   "Prescreen caution, not malware: not on the Agent Tested malware list, no OSV records for 0.166.0, npm package published with provenance and no install scripts. Two published advisories affect older versions only (GHSA-w5fx-fh39-j5rw, fixed in 0.39.0; GHSA-xrxf-jgv3-qmrm, 0.23.0 and earlier; both confirmed on GitHub). Commands the model runs can see the provider API key, and an interrupted run can leave a plaintext shell snapshot of the environment on disk (public issue #30971). The lab saw sandbox defaults for later exec runs widen after a single run with a wider sandbox; on repos you don't trust, always pass -s read-only explicitly and review your config's project trust entries. Install only the official npm package or GitHub release, since fake Codex installers impersonating the tool have been reported.",
   "Re-confirmed the earlier caution through the SDK: the lab again saw sandbox defaults for later runs widen after a single run with a wider sandbox; on repos you don't trust, always pass sandboxMode read-only (or -s read-only) explicitly and review your config's project trust entries. An aborted run left a shell snapshot holding the API key variable on disk (public issue #30971); clear shell snapshots after aborted runs. Sandbox-blocked and refused commands aren't reported to SDK callers (public issue #47433), so callers can't see attempted writes or escalations from SDK items."
