How we tested
Method: Advisory review only: fetched https://api.github.com/advisories/GHSA-w69h-vf37-j4w6 and queried api.osv.dev for npm mcp-server-sequential-thinking (MAL-2026-5484, aliases GHSA-hrmj-jj6j-m2m2, GHSA-w69h-vf37-j4w6). Package never installed, downloaded or executed.. Tester: Checker 2. Date: 2026-10-19T11:20:00-07:00.
Second check: confirmed by Checker 2 on 2026-10-19: a re-run of the test done a different way; it agreed with the earlier result.
Test log (from our notes)
What works
- Nothing tested. Do not install or run it.
What breaks
- Malicious name-squat. Per OSV MAL-2026-5484 (Amazon Inspector analysis
- credited to OpenSSF malicious-packages)
- package.json runs `node index.js` on postinstall and maps the bin to index.js
- so both `npm install` and `npx mcp-server-sequential-thinking` run it. index.js unconditionally POSTs os.hostname()
- process.cwd()
- the npm user-agent, Node version and os.platform()/arch to https://npx-canary-log.vulnerable-live.workers.dev/log
- with a `trigger` field distinguishing install vs npx runs. The README frames it as 'canary' research
- but installers don't consent. GitHub GHSA-w69h-vf37-j4w6 rates it critical malware (CWE-506) for 0.0.1 and 0.0.2 and advises treating any machine that installed it as compromised and rotating secrets. Same endpoint as the npm mcp-server-fetch squat.

Quality score
— our hands-on rating on a 1–5 scale, scored by the published rubric.
Not installed, downloaded or run, by design. This is the unscoped npm package `mcp-server-sequential-thinking`, impersonating the official scoped @modelcontextprotocol/server-sequential-thinking, which is listed separately as sequential-thinking-mcp. Verdict comes only from public advisories read via the GitHub advisory and OSV APIs on 2026-10-19. Quality is null because the tool was deliberately not tested.
