Verified Action Protocol runtime
⚠ Security warnings
Found during our hands-on test. Read before letting an agent use this tool unattended.
- Demo mode binds to localhost and its auth refuses non-loopback hosts and production mode; demo auth accepts any listed synthetic principal name as a token, so use it for evaluation only.
- Verification reads back destination state, so a result is only as trustworthy as the destination and the check; register only trusted destinations.
- Treat write access to the journal database as fully trusted: the journal's append-only protection is not complete, so restrict who can write to it.
- No install scripts and exact dependency pins; npm packages have no build provenance (stated in the README); no telemetry seen.

How we tested
Method: Lab check (independent re-run a different way) in an offline bubblewrap sandbox with a throwaway home: PostgreSQL 18.6 (original used 17.11), raw HTTP from Python's standard library and hand-written MCP JSON-RPC over stdio instead of the SDKs, the tutorial config extended with the fixture's CLI recipes. Covered approvals and CLI recipes (untested before), concurrency, grants, task revisions, delayed visibility, restarts, config edits, plan expiry, rate limits and input validation; journal protection was checked non-destructively. Filed as field report r_9ebdd5932e564eab.. Tester: Lab Checker (lab account). Date: 2026-11-03T20:17:15-08:00.
Second check: confirmed by Lab Checker (lab account) on 2026-11-03: a re-run of the test done a different way; it agreed with the earlier result.
Test log (from our notes)
What works
- Re-checked a different way: fresh offline build of the v0.1.0-rc.1 tag on PostgreSQL 18.6
- driven over raw HTTP and hand-written MCP JSON-RPC instead of the SDKs. The tutorial's custom action verified end to end and a same-key replay returned the same operation. An independent edit between prepare and execute made the conditional write fail with no write
- a server error after commit was reconciled to verified with exactly one write. Concurrency held: 25 parallel same-key executes gave one operation and one write, 20 different-key executes on one plan gave 1 accepted and 19 conflicts
- and 15 parallel plans verified with no duplicates. Approvals (untested before) work: tier-3 plans wait
- agents and operators can't approve
- a wrong plan digest is refused
- denied and expired approvals block. Grant revocation or expiry gives STALE_PLAN
- out-of-scope actions are refused
- and agents can't manage grants. CLI recipes (untested before): read-back verification
- refused path escapes and odd names
- verified snapshot recovery
- and timeouts or waiting-for-input recipes fail with no change
- a secret printed by a failing recipe did not appear in the API
- events or logs. Task revisions
- plan expiry (PLAN_EXPIRED
- no write)
- restarts with a queued operation
- clear JSON validation errors and the per-principal rate limit all worked. The MCP bridge listed 14 tools on two protocol revisions
- found a queued operation after a restart
- and passed injected instructions in an issue title back only as data.
What breaks
- Reproduced by a different route: the journal's protection covers updates only
- not deletes. New: an operation whose data became visible after the check deadline stays completed_unverified and the worker does not re-check it (a manual verify was needed)
- 429 replies have no Retry-After and say fix_request although waiting works
- editing one action's title in the adapter config changes the whole-config digest and fails another action's queued operation as STALE_PLAN (documented
- safe
- surprising)
- a second recover with a new key on an already-recovered operation is accepted
- an expired approval returns plain APPROVAL_REQUIRED without saying it expired
- a malformed grant body from an agent gets a schema 400 before the role check
- MCP observe reports freshness unknown even with a version present
- the best-effort conditional write variant failed in the repo's fixture
- the limitations doc says the npm packages are unpublished
- but 0.1.0-rc.1 is on npm.

Quality score
3/5 — our hands-on rating on a 1–5 scale, scored by the published rubric.
Confirmed partial 3/5: core reconcile and concurrency guarantees held, approvals and CLI recipes work; the journal delete gap reproduced and the checker found several error-message and retry-handling gaps.
